Logansport Memorial Hospital Chooses Cerner to Transform Electronic Health Record and Manage Health of Populations
KANSAS CITY – April 22, 2019 – Logansport Memorial Hospital (LMH), a not-for-profit regional medical center in north central Indiana, has teamed up with Cerner to transform its electronic health record (EHR). LMH will also put in place solutions that will help predict and manage the health of patients, through a population health strategy. LMH physicians, nurses and staff will have advanced medical technology to support better health care experiences and outcomes for the people in the communities it serves.
“Having all of a patient’s information on one record, rather than working from multiple systems, will be hugely beneficial for our physicians and staff as we continue to improve the health of people across the region and make a positive difference in the lives of those we serve,” said Perry Gay, president and CEO, LMH. “Our shared vision with Cerner will guide us in managing the health of our patient populations and inform care moving forward. We are excited for what the future holds for Logansport and our surrounding communities as we continue to grow and improve care.”
LMH chose Cerner as the partner of choice for health care innovation to transform the care it delivers throughout the community and create a more seamless, connected experience. The Cerner Millennium® EHR platform provides physicians with an integrated digital record of a patient’s health history that includes clinical and financial data. A digital record that brings together each patient’s health information from different venues of care will benefit physicians and nurses with an up-to-date view of a person’s well-being. Through a new online portal, patients can securely message doctors, schedule appointments, view and settle balances and access their health history. LMH will also implement Cerner revenue cycle solutions to help reinforce the health system’s financial operations and connect clinical and financial information for staff to view together in one patient record, supporting a quick and efficient patient billing process.
LMH will also adopt a population health strategy to improve the health of the entire community, implementing Cerner’s HealtheIntent® big data and insights platform for population health management. The platform analyzes medical data across the health system and supports physicians with actionable insights to help engage people beyond the hospital setting and manage health outcomes. LMH will use the technology to support improved management of the groups of people it serves, designed to help the organization know and predict what will happen within its community to guide care. Implementing an intelligent IT system and technologies will help LMH drive innovation and reduce complexity in order to provide the best possible care they can for patients.
“As LMH grows and navigates the changing health care landscape, it is important that they have the proper IT infrastructure to support their future development,” said Mitchell Clark, president, CommunityWorks, Cerner. “With the support of our technology, LMH will provide high-quality, connected care that improves the patient experience and engages people to take an active role in their health. It is our commitment to relentlessly advance our clients’ successes, and we plan to support LMH as they innovate to make better health care a reality in their community.”
LMH will leverage Cerner CommunityWorks℠, a cloud-based delivery model that is tailored to help community, critical access and specialty hospitals to securely share patient health history, including clinical and financial data, across multiple venues of care. Built off years of industry and client recommended practices, the deployment model aims to help address the unique set of challenges faced by patients and care teams at these hospitals.
